Taylor Swift has released the music video for “Patient Zero,” unveiling the visual during the 2026 MTV Video Music Awards on Sunday, Sept. 27. The video, which Swift directed herself, stars Dakota Johnson and Colin Farrell and features cinematography from Oscar-winning filmmaker Emmanuel Lubezki.
The video follows Johnson as a woman caught in a troubled marriage to Farrell’s character, while Swift appears as his former partner. Swift’s character initially takes on an almost ghost-like role, appearing around Johnson and guiding her as the story develops. The visual eventually reveals that Farrell’s character has been secretly poisoning the women around him, providing the central twist to the storyline.
Swift had teased the video in the days leading up to the VMAs, with early footage hinting at a death involving characters played by Johnson and Swift. The previews also showed the two women appearing in similar hairstyles and clothing, while a tombstone featured in the visual added to the mystery surrounding the song’s storyline.
“Patient Zero” was released Sept. 25 as one of four additional songs on The Life of a Showgirl: The Encore. Swift had announced the single earlier in the week before confirming that its official video would premiere at the VMAs.
The collaboration with Johnson marked another project between the longtime friends. Johnson previously spoke about working with Swift on the visual, saying they worked well together, while Farrell joined the production as the male lead.
The video also arrived on a major night for Swift at the VMAs. She received the inaugural Artist Director Honors, an award recognizing artists whose work behind the camera has expanded music-video storytelling. Swift was also nominated in 11 categories at this year’s ceremony.
Following its television premiere, the “Patient Zero” video became available to watch on Spotify, Apple Music and Amazon Music. Billboard reported that the visual was not initially released on YouTube, making those platforms the available destinations for the full video after its VMAs debut.
The release adds a cinematic visual chapter to Swift’s The Life of a Showgirl era, with the video using a relationship drama, supernatural imagery and its poisoning reveal to expand on the themes introduced by “Patient Zero.”
